PHS Transcripts

Please complete a transcript request form.  This form can be downloaded at www.gophslions.com by clicking on Forms in the Intellectual section on the home page.  Official transcripts must be mailed directly to a school or employer and will not be given to "hand-carry."  Transcripts are not faxed.  Unofficial transcripts will be marked as such and may be sent to the student or former student.  Please provide complete addresses where the transcript is to be sent. 

 A fee of $5.00 per copy - Cash or Money Order (No checks) - must be paid prior to processing any transcript requests.  Please allow three (3) to five (5) working days from receipt of the transcript request for processing of transcript requests and several more days for the mail to be delivered. 

Students should not assume that transcripts have been sent.  Approximately two and a half weeks after submitting a transcript request to PHS, contact your college (not PHS) to see if the transcript has been received.   Please understand that colleges have a procedure and timeframe for processing transcripts, due to the quantity that they receive.
