Enrollment Information
What you need to register your child at Prattville High
School:
Hint: It is advantageous to call ahead for an appointment;
however, it is not required.
REQUIRED:
Complete mailing address of previous school (and fax number, if
possible)
Withdrawal form from previous school
Proof of Residency and Guardianship
Address verification is court ordered for Autauga County School
System. Please submit a copy of any two (2) documents listed
below from two separate categories. (Please note that any
document with a post office box cannot be accepted.)
Property tax records, indicating homestead exemption
Mortgage Documents or property deed
Residential (apartment or home) lease
Utility bills - electric, gas or water (cable, telephone &
cell phone not acceptable)
Automobile registration
A court decree declaring the resident to be the legal guardian
of a student
If the student lives with other than both parents, official
proof of primary custodial parent or guardianship is also
required. (Copy of divorce decree, court order, etc.)
Custody papers
Birth Certificate
Social Security Card
Discipline Clearance
Picture ID
Alabama Immunization Certificate (Blue Card)
This can be obtained at the Autauga County Health Department in
downtown Prattville, 361-3743, or Maxwell AFB. We can not
accept anything other than the Alabama Blue Card.
Unofficial Copy of Transcript (Required)
This helps us to place your child in the appropriate
class. This will show the credits he or she has already earned
or needs. We request official transcripts from the
transferring school.
Copy of withdrawal papers and withdrawal grades
This will complete the information needed to place your child in
the appropriate courses.

to make an appointment for
registration with a guidance counselor.
